Supplement or Advantage on the Central Coast

Medicare Advantage networks in California are set county by county. On the Central Coast that single fact decides the question for most people.

The network is the decision

Nationally, the Medicare Supplement versus Medicare Advantage argument is usually framed around premiums and out of pocket maximums. On the Central Coast it is settled by something far more concrete: which doctors and hospitals are in which network, this year, in your county.

That is not a rhetorical framing. Santa Barbara County runs about 20.5 percent Medicare Advantage penetration against a California average of about 51.2 percent, and it has stayed flat rather than climbing the way most of the state has. Roughly four out of five Medicare beneficiaries here are on Original Medicare, which in practice means a Medicare Supplement and a standalone Part D drug plan.

People sometimes assume that gap reflects a local preference or a conservative population. It mostly reflects supply. Building a Medicare Advantage network requires competing medical groups to contract with, and this county has very few.

The county line matters more than the state line

What that produces Thin Advantage competition, low penetration, and a market where the supplement is the normal answer rather than the premium one. Dense Advantage competition and penetration much closer to the state average.
If you move between them Moving out of a plan service area creates a special enrollment period, so a move across the county line is a Medicare event, not just a change of address. The same applies in reverse, and the plan you liked in Ventura may simply not be sold in Santa Barbara.

What happened on January 1 2025

Effective January 1 2025, Blue Shield removed Sansum Clinic from the network of its Medicare Advantage HMO. Roughly 3,200 local people found that the clinic they had used for years was no longer in network on the plan they had already chosen.

That is the risk in a concentrated market stated as plainly as it can be stated. Nobody did anything wrong. A contract between an insurer and a provider group did not renew, and thousands of individual coverage decisions were invalidated by it.

There was a remedy, and it is worth knowing about: a loss of network of that kind opens a guaranteed issue window of 63 days during which affected members could buy a Medicare Supplement policy without medical underwriting. Guaranteed issue rights of this sort are narrow and time limited. If a network change hits you, the clock starts immediately and it does not wait for the autumn.

Write down your providers before you look at a single plan

This is the exercise we run with every client here, and it usually answers the question inside ten minutes. Before comparing anything, list the specific names you intend to keep.

  • Sansum Clinic. For a very large share of this county, the primary care doctor and most specialists sit here. Ask which Advantage networks currently include it, and remember that the answer can change at a contract renewal.

  • Cottage Health. Santa Barbara Cottage Hospital, Goleta Valley Cottage Hospital and Santa Ynez Valley Cottage Hospital. Check the hospital as carefully as the doctor. People check the doctor and forget that the admission is the expensive part.

  • Community Memorial. A significant system in Ventura County, and relevant if your care crosses the county line or if you split time between the two.

  • CenCal Health. The county organised health system that administers Medi-Cal for Santa Barbara and San Luis Obispo counties. If you qualify for both Medicare and Medi-Cal your options and your enrollment rights are different, and worth a separate conversation.

  • Anyone you would not want to leave. The oncologist, the cardiologist, the surgeon who did the last operation. Write the name down.

With a Medicare Supplement there is no list to check. Any provider in the country that accepts Medicare accepts your supplement, and there is no network to be removed from. That is precisely what people in this county are buying.

Where Advantage genuinely is the better answer

We are not against Medicare Advantage, and we will recommend it when it fits. It fits when the supplement premium does not fit comfortably in the monthly budget, because a plan with a low or zero premium and copays as you go is a real answer to a real constraint. It fits when the network happens to contain every provider you use and you are content to check that again each autumn. It fits for some people who value the extra benefits bundled into these plans.

What we ask people to understand is the asymmetry. Moving from a supplement to an Advantage plan is straightforward and can be done in any Annual Enrollment Period, October 15 to December 7. Moving back the other way, from Advantage to a supplement, normally requires answering medical questions, and the application can be declined. There are exceptions, including the twelve month trial right the first time you ever join an Advantage plan after dropping a supplement, and the California Birthday Rule for people already holding a supplement. But the general direction of travel is one way.
